Brief summary
Emergency abdominal surgical pathology occupies a leading position in the structure of hospital mortality, and acute appendicitis is one of the most common urgent abdominal diseases worldwide that require surgical intervention.
Objective. The aim: to study the dynamics of the performance indicators of medical organizations in St. Petersburg in providing medical care to patients with acute appendicitis for 2005-2024.
Materials and Methods. The data of monitoring the state of acute surgical pathology of abdominal organs were used in accordance with the Order of the Health Committee of St. Petersburg dated 31.07.2009 No. 477-r "On the control of emergency hospitalization in state health care institutions of St. Petersburg", conducted by the St. Petersburg Research Institute of Emergency Care named after I.I. Dzhanelidze, 2005-2024. Statistical and analytical methods were used.
Results. The 20-year observation period recorded an almost stable annual number of patients with acute appendicitis in St. Petersburg (about 5 thousand people) with an operational activity of 99.5±0.3%. At the same time, a high proportion of this pathology in the structure of acute surgical pathology of the abdominal organs was noted (29.1±5.2%) with a pronounced stable tendency to increase to 36.5% in 2024. Analysis of the timing of patient hospitalization revealed negative facts of late hospitalization: 61.8% - on the first day and 38.2% - later than 24 hours. By the timing of surgical treatment: 69.8±3.9% - in the first 6 hours from the moment of hospitalization; the remaining 30.2% required dynamic observation, differential diagnostics. The average postoperative mortality rate was 0.12±0.06%, with a decrease of 0.1% over the observation period from 0.2% in 2005 to 0.1% in 2024.
Conclusions. Acute appendicitis accounts for a significant (more than a third) share of acute surgical pathology of the abdominal organs. Late hospitalization is one of the main causes of unfavorable outcomes. The availability and quality of medical care are the main principles of health protection in the Russian Federation. They should be ensured, among other things, by health education work among the population, improvement of the organization of primary health care and emergency medical care, which will facilitate timely diagnosis of this pathology, preserving the health of the country's population.
