Антифосфолипидный синдром

Резюме
Антифосфолипидный синдром представляет собой системное аутоиммунное заболевание, связанное с тромбозами сосудов и/или повторными эпизодами гибели плода в присутствии устойчиво повышенного уровня антифосфолипидных антител. Основной антигенной мишенью для этих антител был идентифицирован β2-гликопротеин I, который выступает посредником для связывания антифосфолипидных антител с клетками – мишенями, включая эндотелиальные клетки, моноциты, тромбоциты и клетки трофобласта, что приводит протромботическим и провоспалительным изменениям которые, в конечном счете, могут привести к развитию тромбоза и/или гибели плода. С момента создания международной предварительной классификации и критериев диагностики антифосфолипидного синдрома в 1999 году был выполнен значительный объем работы в области фундаментальных исследований, позволивший получить новую информацию. Клинические критерии антифосфолипидного синдрома были пересмотрены в Сиднее в 2006 году. Само по себе присутствие антифосфолипидных антител не означает развитие у пациента антифосфолипидного синдрома. Для диагностики антифосфолипидного синдрома требуется комбинация, по меньшей мере, одного клинического и одного лабораторного критерия. Критерии Сиднея не рекомендуют использовать термин «вторичный антифосфолипидный синдром». Выполненные исследования не смогли найти различия в клинических последствиях действия антифосфолипидных антител у пациентов с первичным и вторичным антифосфолипидным синдромом.
