Diagnostic informativeness of plasma cell determination by histological and immunohistochemical methods in the diagnosis of chronic endometritis: comparative roc-analysis.

The question of diagnostic informativeness of plasma cells (PC) in establishing the histological diagnosis of chronic endometritis (CE) is the "cornerstone" of many years of discussion. This paper presents the results of a comparative evaluation of the informativeness of the PC as a diagnostic indicator determined by two methods: histological (Hist) and immunohistochemical (IGH). The study group consisted of 937 women with infertility, failed IVF attempts and established diagnosis of CE. The control group consisted of 103 healthy women who were examined for male infertility factor. ROC-analysis showed that based on only one diagnostic criterion - the determination of plasma cells in the endometrium, a diagnosis of CE can be established only in 5% of patients by histological method (sensitivity -Se = 4 5 7%), informative value - likelihood ratio LR[+] = 1,5 5,7 220 and LR[-] = 1,0 1,05 1,07 and low prognostic value with positive result (PPV = 2 10 46 % with Prev = 1.0 2.0 3.0% PPV = 16 59 91% and with Prev = 10.0 20.0 30.0 %) and 68% of patients by immunohistochemical method (sensitivity -Se = 65 68 71%), informativity -LR[+] = 19 71 2790 and LR[-] = 2,8 3,1 3,4 and high prognostic value -PPV= 17 59 91% at Prev = 1,0 2,0 3,0 %; PPV= 71 95 99% at Prev = 10,0 20,0 30,0 %).
The low quantitative value of PC in chronic endometritis (1 cell) cannot reliably reflect the degree of inflammation activity. Thus, the histological diagnosis of CE should take into account a complex of morphological criteria that reflect the chronic inflammation.
