Комплексное лечение карциноматоза брюшины при колоректальном раке

Резюме
До недавнего времени карциноматоз брюшины при колоректальном раке считался фатальным исходом для подавляющего большинства пациентов. Однако два новых метода лечения существенно улучшили результаты лечения этих пациентов. Новый хирургический метод лечения представляет из себя циторедуктивную операцию дополненную перитонэктомией. При выполнении этой процедуры выполняется резекция всей макроскопической опухолевой ткани с таким расчетом, чтобы оставались только микроскопические очаги опухоли. Интраоперационная внутрибрюшинная химиогипертермическая перфузия, является второй относительно новой методикой, которая применяется с целью уничтожения остаточной микроскопической опухолевой ткани. При необходимости, в течение первых пяти дней после оперативного лечения, может проводится внутрибрюшинная химиотерапия. Комбинированное использование методов циторедуктивной хирургии и внутрибрюшинной химиотерапии улучшает показатель выживаемости и оптимизирует качество жизни данной категории больных. Данная статья описывает историю развития метода внутрибрюшинной химиотерапии с гипертермией, преимущества и недостатки внутрибрюшинной химиотерапии. Так же описывается процедура перитонэктомии. Освещаются вопросы механизма действия интраоперационной внутрибрюшинной химиогипертермической перфузии, дается клиническая оценка метода, критерии отбора пациентов, которым показан данный комбинированный вид лечения. Представлены результаты комбинированного лечения для пациентов с колоректальным раком.
