Резюме
Острый бронхообструктивный синдром (ОБОС) относится к числу наиболее тяжeлых проявлений респираторной патологии и сопровождается дистальной обтурацией дыхательных путей, развитием ателектазов, выраженным вентиляционно перфузионным несоответствием и острой дыхательной недостаточностью. В ситуациях, когда стандартная терапия и традиционный бронхоальвеолярный лаваж (БАЛ) физиологическим раствором не обеспечивают достаточной санации нижних дыхательных путей, особый интерес представляют технологии, сочетающие устранение обструкции с поддержанием дыхательной функции лeгких. Одним из таких подходов является БАЛ с перфторуглеродами (ПФУ). В обзоре систематизированы экспериментальные и клинические данные о применении БАЛ с ПФУ при тяжeлом ОБОС у взрослых, детей и новорождeнных. Рассмотрены ключевые физико химические свойства ПФУ — высокая плотность, низкое поверхностное натяжение, значительная растворимость респираторных газов, химическая и биологическая инертность, — лежащие в основе многокомпонентного механизма действия метода: механической эвакуации патологического содержимого из дистальных отделов дыхательных путей, альвеолярного рекрутирования, частичного поддержания газообмена и улучшения механики дыхания. Показано, что БАЛ с ПФУ является самостоятельной санационной технологией, отличной от частичной жидкостной вентиляции (PLV) по цели, характеру вмешательства и клиническому назначению. Наиболее убедительные клинические данные по ПФУ БАЛ на современном этапе относятся к массивному лeгочному кровотечению с формированием дистальных тромбов и длительным ателектазам, резистентным к стандартной терапии на фоне глубокой мукообструкции. При альвеолярном протеинозе и других протеинозоподобных состояниях ПФУ технологии демонстрируют преимущественно функциональный эффект и могут рассматриваться как вспомогательный компонент комбинированных схем, но не как альтернатива тотальному солевому лаважу. Вместе с тем патофизиологические предпосылки метода позволяют рассматривать его как потенциальную опцию и при других формах тяжeлого ОБОС, включая острые пневмонии с выраженной дистальной мукообструкцией и аспирационные поражения, хотя клинические данные для этих сценариев пока остаются ограниченными. Современная доказательная база ПФУ БАЛ представлена преимущественно отдельными случаями и небольшими сериями наблюдений; методика остаeтся нестандартизированной, а результаты варьируют между центрами. На данном этапе БАЛ с ПФУ следует рассматривать как перспективный, но специализированный rescue подход для тщательно отобранных пациентов с тяжeлым ОБОС, резистентным к стандартным методам лечения. Дальнейшее развитие этого направления требует стандартизации методики и проведения систематических клинических исследований, позволяющих уточнить показания, ограничения и соотношение ожидаемой пользы и потенциальных рисков.
