The state of oxidant-antioxidant system in patients with acute myeloid leukemia

Brief summary

Aim. To investigate the state of oxidant-antioxidant system (OS-AOS) in patients with acute myeloid leukemia (AML) before and in the course of treatment. Material and methods. We included 39 AML patients (17 men and 22 women, mean age 50,0?8,0 years) in the study. The group of 13 patients was treated by consolidating course of high-dose chemotherapy (HDC) with a single dose of cytarabine (≥ 1g/m2). Induction course of therapy in the format «7+3» was performed in 13 patients. The autologous hematopoietic stem cell transplantation (autoHSCT) with conditioning by busulfan plus cyclophosphamide was used in other 13 patients. The blood plasma content of malonic dialdehyde, the secondary product of lipid peroxidation, was measured to indicate the presence of oxidative stress. Superoxide dismutase and catalase activities, cerulloplasmin level and erythrocyte content of non-protein thiol groups (NTG) were analyzed to characterize activity of antioxidant system. The parameters of OS-AOS were determined four times in each group of AML patients - before treatment, immediately after initiation of treatment, at the time of maximal cytopenia and upon reconstitution. Results. It has been demonstrated, that in AML patients the features of imbalance of OS-AOS are present, most pronounced in the state of deep cytopenia, with all the studied indicators not reaching normal values after treatment. After the use of autoHSCT or HDC, the indicators of OS-AOS, except for NTG content in erythrocytes, approached the normal levels. The sole use of induction therapy did not cause the same effect. Conclusion. The imbalance of OS-AOS in AML patients is not fully eliminated during their treatment. Further studies are needed to assess both necessity and possibility of OS-AOS imbalance correction in this group. The level of NTG in erythrocytes could serve as an important biomarker of disease state during the treatment of patients with AML.
