On the basis of morphometric measurement in an axial projection by means of a CT and/or MRI of a brain of bitemporal distance, width of a tentorium, diameter of a foramen magnum and a mutual ratio of these parameters in points suggested to estimate quantitatively an anatomic intracranial reserve as minimum (3-4 points), average (5-7 points) and big (8-9 points). For the developed way of definition of an anatomic intracranial reserve the patent for the invention No. 2517767 (Shchedrenok V.V. et al., 2014) is taken out. At approbation, the way showed high precision (92%) and predictive value (85%) in determination of terms and volume of surgical treatment of 150 victims with a craniocerebral injury and 100 patients with primary tumors of a brain. The offered way showed also enough high diagnostic importance (89%) in the forecast of complications and results of surgical treatment.
